Health History Form

All full-time students must submit a health history form before entering the first semester. This form is obtained from the University health center, the health center website, or the admissions office. The University also requires completed immunization records, including documentation of measles immunity (e.g., providing proof of having received two doses of MMR vaccine) in compliance with Oregon state law. Current tubercular testing is also required for all full-time international students. Information and vaccinations are available at the University health center website.

Persons 18 years or over may assume responsibility for their own health care in the state of Oregon.
